Why my facebook page is not working on mobile device?

When the page is viewed on the mobile phone m is appended in the URL, which is normal, but the page is not working:

It is related to some Facebook bug, I found this article about that issue: https://developers.facebook.com/bugs/1051463851558493/ please check and try to whitelist your app (according to that article). So it has nothing to do with the form actually. If you try to view the page without appending m it will work, on mobile as well.
