Stuck on please wait when submitting form

Having issues with this form again. We only use it once every few years, but it needs to work when we do use it. When you click submit is just says "please wait" and nothing happens. There is NO submission recorded.

I have tried submitting your form and see that the Submit button is stuck with Please wait message after submitting the form. As I check the form further, I see that the form is having the Smooth signature widget which is not configured properly.

You may consider deleting this widget from the form. Also, the other signature widget on the form seems not to be working correctly. Please try removing this widget and add it again to the form and see if that helps.
