We have switched to the free plan around 6 months ago but our card is still being charged $19.00 monthly.

I have checked the account fpsrose, this account is in Free status, we do not have records of any subscription payment.

That account's last charge was on 7th of November 2016. So no recent charges were made.
And you can check that in the billing page: https://support.jotform.com/myaccount/billing while logged into that account.
