Your account is suspended due that you are requesting bank account details in your form.
We have removed the guilty fields on this form and reactivate the account. You must refrain from doing a new form like that to prevent a potential suspension of your account.

thanks for your quick response. Bank account details are absolutely needed, because the website is about helping people who have trouble with their bank accounts. Without receiving the needed informations the form is unusable for me. I wonder how it is possible to receive bank account details via other contact forms, so how can the problem be solved?
I need the fields "Bankverbindung" and "IBAN" at the beginning of page 3. Could you please add them after making technical or data security arrangements? At least I need a detailed instruction for solving this problem by myself (if possible, please in german). Thanks for your efforts.

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.
If it happens that your account becomes suspended again because of these fields on your form, it will be difficult for us to re-activate your account again in the future.
