How To Change Format of Date & Time Widget on Form

I do not like the format of the date/time picker widget on my form.  While reading a Jotform Support Forum, l see a form using the format that I would desire for my date/time picker widget.  The form is located at https://form.jotformeu.com/71121916984359.

Can you please advise how to accomplish this on my form?
