How can I create a multi page form?

    I am trying to create a short form to get some initial information and then on submit it would take them to a page that ask for additional information if they wish to provide it. If they do not provide the additional information  we would still receive the initial submission content.

     

    INITIAL PAGE 
    Name
    Email
    Phone

    On submit
    Address
    and other information

    Hello,

    Thank you for contacting us.

    You can create a multi-page form, you can get initial information on the first page. Then, you can make this fields as a required. Then, you can get additional information on the second page but these fields must be non-required. So, if they wish to provide it, they can be fill. 

    The submit button have to be on the second page.
