Our form randomly stopped functioning correctly

    Hello,

    Customers should be able to fill out this form and reserve a dock slip. However, it seems the fields are being confused by the system somehow?

    Customers cannot enter their credit card number or any other information without the form telling them that it is incorrect and asking them to instead insert a valid email, which is not necessary as there is already an email portion of the form.

    Above is the URL of the actual page, and I have included a picture of what it looks like when the form is demanding the incorrect information.

    You are using an Email field for the Account and CVC Code.

    You need to use the textbox or the number field for these types of fields. However, our terms of use prohibit this type of form. We do not allow forms that could collect a credit card information using our plain text fields. You can check our terms here: http://www.jotform.com/terms/
