Hi There, we're having an issue with an embedded form. It doesn't seem to be loading properly on Chrome version 50 and higher. Can you take a look and see if there's something going wrong here?

I have checked your web page and see that the form is not displayed on the page. It seems that there is a conflict with the other scripts on the web page. Could you try embedding the form using iframe embed method to see if that works? Please refer to the guide below that can help you with getting the iframe embed code for your form.

Hi Kiran, I have checked for conflicts on the page and removed the one possible conflict I saw. I have also switched the form to using the iframe version.
Some additional info is that we have been able to use jotform successfully in other places of our site.

We would like to apologize for any inconvenience, however, can you try removing the <script> part of the iframe embed code and see if that would solve the issue you are having.
I would also suggest you adjust the height attribute of the iframe to match the height of the form.

Looks like that was indeed the issue. The form now shows up properly on all the browsers we tested. Thanks for the help.
