I am not able to revert back your form to earlier revision.

    I went into advanced designer. Made width of form a little wider, went back to "build form" and now its completely deleted! I tried to revert to earlier forms and it shows nothing when previewed except for my background image! HELP!

    The form that is no longer showing was called INSTANT QUOTE REQUEST. It was working perfectly fine on the 28th April before Approx 11.53pm

    I'm sorry for the trouble caused to you. 

    I did check your form's revision history and found that you have deleted fields from your form. It also displays revision made on Jan 19, 2017 which shows your complete form. Please check the screenshot below:

    I would suggest you to please revert back your form to Jan 19 revision.
