I need confirmation that my form adheres to the terms of use of Jotform

Can you please review my 

Online Reservation Form

and let me know if all the fields including credit card are acceptable by term and condition of Jotform?

We provide service and require a deposit for our service before reserving a vehicle for damage deposit. Please review and provide your feedback if credit card information on our form is acceptable.

Yes, the form is good. For as long as the payment integration is used, you should be fine. The only forms that we do not allow are those forms created to collect a credit card information or login and password or any form that would collect a sensitive information.

Keep an eye on your reservation form.

I created an online reservation form last year for the campground I work at. Our form is designed for the customers to enter their credit card information at the bottom like your form. I could not tell if you used an online credit card processor or not. We do not process the credit cards online, rather here in the office instead. Lasy year I was told by Jotform and Security Metrics that the form was ok to use. The form and our web site is PCI compliant.

This week Jotform has removed a protion of my online reservation form where the customers enter their credit card information. They stated in the e-mail that the fields were suspicious of phisihing activity according to their automated Phishing detector system. Jotform wants me to use one on the online credit card processors instead.

Valerie, please note that, as my colleague Welvin found and stated, Shawn is using a payment integration for his credit card use, and as a result he shall never have any problems at any point in time, with this form.
