I am unable to log in to my account. The email address associated with the account is integratedcare@msm.edu. I have logged in several times this week, and when I logged in a text box came up saying that the email was on a bounce list. I tried the password reset feature, but now it says that the email is not associated with a JotForm account. Please call me at (404) 756-5044.

I have checked using the form you shared and found the account, the email address is indeed different: i********d.c**e@msm.edu
It was also in our bounce list, but I have already removed it so you can reset your password now.
