How do you get the documents to attach it self to the terms and condition link successfully?

Hello, you failed to mention which form you're working on so I'll just assume it's this one https://www.jotform.com/71174458949167
I saw a Terms & Conditions Widget at the bottom of that form and noticed the link is broken. The reason it's broken was because you never a placed a link on the widget. What you placed were the following texts:
Participating Child or Adult does not carry medical insurance for its students enrolled in the clinic/classes. It is required that all students be covered by their own family insurance policies and if injury occurs it is understood that the student's own policy is your only source of reimbursement
So, just ensure you will put a link in the TERMS LINK section of the widget (e.g. http://www.jotform.com/terms/) so your users can click on it to read your Terms & Conditions.

The screenshot I sent was marked with numbers where you need to click to in order to setup the link to your T & C. Here's a short clip showing that same principle:
I used our own Terms Page link http://www.jotform.com/terms/ in the example above to show you how to link a Terms page with the widget. So just do the same from your end and use your own website's Terms and Conditions link.
