    The name on my forms still shows my old name, not the current one

    I have requested to have my forms say "center for adhd" on them.  They still say "center for social".  Despite the fact I changed my name.  Please make this change so they say "Center for ADHD".

    I can see that my colleague has already guided you on how to change the form links of your form in this thread: https://www.jotform.com/answers/1117350.

    However, when I checked your forms in your account, I noticed that the form links are still in the default link. Please make sure that you have clicked the "Apply" so that the changes will be saved when you customize the form link.
