How do I turn off the ID and IP section?

nobody else uses this form except me so i dont need the IP because its mine and i also dont need the ID portion and it seemed to add that in there on its own it wasnt there now it is 

You can disable that on the Submissions page. Here's how:

1. In My Forms page, select the form and then click the "Submissions" button.

2. On the Submissions page, click the "Properties" icon. Uncheck the "ID" and "IP". After that, click the "Properties" icon again to save the changes.
