    Hi,

This is because your form height is greater than the container and also you are using script embed code which automatically increases the container height. Is iframe embedding ok with you? If it's fine, you may embed your form using this iframe code:

     <iframe allowtransparency="true" src="//www.jotform.us/jsform/21894515226153" frameborder="0" style="width:100%; height:465px; border:none;" scrolling="auto"></iframe>

Just change the iframe height (bolded part) if it doesn't fit exactly on your page. Hope this works for you. Thanks!

No, just remove the first embed code on your page and replace it with that code.

Let me know the result so I can also check your page here. Thanks!
