Form submission not working

Hi,

I have the following form on Jotform:

This evening multiple users have been reporting that they are unable to submit the form.

When I check, I observe that on hitting Submit, the form displays a "Please Wait" message but does not get submitted.

Please help because there are multiple reports stuck due to this issue.

I just tested your form, and it was submitted without a problem, the only thing I noticed is that the submit button is labeled as "Preview", and that may confuse the users:
