Where do the forms go after someone fills them out?

    Hello,

    I did a practice run and only got an email copy... can I get these forms to be in a word or pdf that I can upload into my patients software files?

    When form is submitted submitted data will be sent to our servers. Like you mentioned when form is submitted you will receive email Notification with submitted data. You can also view submitted data on your JotForm account.

    You can also attach PDF report of your submission to email Notification.
