Why is my form disabled again?

    why my form be disable again. please help me..urgent

    Your account is autosuspended by our phishing system because of your form with ID 71206081447451.

    Inside of this form you are asking for user login credentials (password) and that is in violation of JotForm Terms of Use.

    Phishing - You agree that Company may terminate your JotForm Service immediately If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.  

    I have deleted password fields from your form and I have activated your account. Please do not create similar forms or your account will be suspend again and we will not be able to activate it.

    Thank you for your replied. Between that, I still have a question which regarding to a log-in form creation that contain of several digits and security numbers. So, it is available that I still can create one log-in form with security numbers?

    What do you mean by security numbers? Here's a snippet from our terms of use:

    You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features.

    As long as your form complies with our terms of use, then you won't have any problems creating it. Hope this answers your question. Please do get back to us if you need any further help.

    I want to create a login registration form that have contain password in my create form.  But the password was user login credentials/security numbers. So, it is available that I still can create one login form with user login credentials/security numbers (password) ?

    Between that, How to create a login form that have to use best security practices of jotform including SSL and Encrypted Forms features?

    Unfortunately, you can not create login form anyhow as it is noted in our Terms of use:

    Our Forms are SSL secured by default so you don't have to do anything related to that. Regarding the Encrypted forms here is a useful guide: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

    If you need to collect Social Security Numbers you can do that, as my colleague explained, but you need to make sure that the data you're collecting is secured and that your users are willing to provide the same which you can ensure by adding the e-signature widget, you can check them here: https://widgets.jotform.com/search/signature
