I get in the final "Form not found"

    We apologize for the inconvenience, your account was suspended by our phishing detector system. I checked your form, and it does not violate our terms of use, however, the phishing detector is not updated to scan forms made in Portuguese language.

    I have white-listed your form to prevent this from happening again, however, it would be good if you could implement the following workaround to make sure it won't happen again: https://www.jotform.com/answers/447134-Form-in-Arabic-language-suspended-, which is basically translating your form to English: https://www.jotform.com/help/298-How-to-Make-your-Forms-Multilingual
