How do I transfer an existing shared form to my account for editing from another account?

So I have recently updated my account and I was using a shared job application form, I need to know how I can transfer the job application form so I can edit it on my account from smakmobi.  They have shared the file but I cannot update or edit anything.  I also want to keep it linked to my website, thanks.  We recently took over the website and want to keep everything as it currently is.

The form has been shared with you, but you have not access to edit the form, you can actually only view the submissions received on that form. 

The form's owner will need to go to the Users section on My Account page in order to edit the permissions given to your account.

Now, if the form needs to be transferred from smakmobi to jkolb1 then we will need to get the request from the form's owner, on this case it should be from "smakmobi".
