Do you offer a form/template that I can send out to customers that they can fill out and send back without being identified?  I want to get some feedback on a product/service of mine please... 

Yes, you can indeed only create your form or edit it from a template and ask only for the questions you need, it is not necessary to ask for personal or contact information.
