    Hi,

The form using the direct link is accessible, you can test it out in its direct link here: https://form.jotform.co/70800791435859. The most likely issue that the form is not properly showing up on a user's mobile device is because the website is not handling properly the form on a mobile view. You might need to consult with your web developer about this.

I did check your website and I found out that in mobile view, there seems to be a jQuery window that handles that form's iFrame embed code, is that correct? It seems like the parent element is not responsive by itself, that is why the form is not properly being displayed in mobile devices.

To make the form itself responsive, you can also check this guide: https://www.jotform.com/help/322-How-to-make-mobile-friendly-forms-on-JotForm

But for your specific case, the website itself needs to be able to handle the embedded form properly, either by using Javascript or using CSS. I am not familiar with your website builder or on how the responsiveness is implemented so its best to consult with your web developer/designer.

    I hope that helps.
