Why is my form disabled?

    Haven't used my account in a while, and doesn't seem to be working now that I'm back.

    Hello ,

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Have a Password to Get in?" has fields like "Password" which makes it suspected of being used for phishing activity.

    Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the suspected form and activated your account again. Do try to sign in to your JotForm account and get back to us if you have any trouble.

    Kindly refrain from creating form with email/password, login/password or credit card details.
