How do I upload my form to my Facebook business page?

Hello,

I tried to upload one of my JotForm's to my Facebook business page. After uploading, one of the JotForm features, but after going to my Facebook page, I did not see the app.

Has something changed? Where do I go on Facebook to install the app?
