I have a pdf document I'd like to upload. It's only giving me the option of using an existing document or a url. I want to upload it from my desktop.

I wasn't sure if you want to embed it in the form or attach it somewhere?

Unfortunately, we don't have a tool that can convert a PDF form to an online form. This tool is currently not working. Also, that tool was created for Adobe FormsCentral users.
Please use our Form Builder to create an online form from scratch. Here's a helpful guide: How-to-create-your-first-web-form.
After that, you can convert it to a fillable PDF, here's how:
In My Forms page, select the form and then click the "More" button. After that, click the "Create PDF Form" option.
