    Not all fields are submitting email data

    My form 51347150259958 email submissions are not showing three fields. 

    I have looked at the field names a number of times and I don't see an error.

    Can you advise what is wrong please.

    The fields data not being emailed are;  

    Name of host club{nameOf},

    Date of:{dateOf19},

    I checked the notification emails settings and it is configured properly. I cloned your form then I did a test submission, I was not able to replicate the issue. 

    Are you referring to the custom Thank You page? If yes, please try changing the Fields to your own preferences. 

    Here's the changes I made on my cloned form:

    I will look into this also.. Although it was not what I was referring to.

    The last email I received was missing the following:

    (Note that I modified the Q column names today and are no longer as shown)

    I will wait for the next submission and advise. I will change the "thank you" page as shown.
