I need to create an online form based on an existing 4 page PDF for job application.
The application itself is 3 pages and the last page is a release document which needs to be signed and returned with the application.
What is the best way to implement this? Should i just layout 1 long page with all of the fields or implement some type of multi-page form?
Besides usability for the person entering data online, i need to make sure the recipient gets the data and signed document back.
I'm just not clear on what is supported/available or what the best practice might be?

You can create the multiple pages form using the Page Break field.
As for the last page with document signed requirement, you can use e-signature integration available.

You can use the Autoresponder Email for this(which I see you have already added on your form)
You can include attachment file in emails.
