PDF Fillable Form: Why questions are cutting off?

    Hello,

     

    I just signed up with JotForm, and its incredible!

    I just wanted to ask a few questions, when I try to download the PDF version of the form it seems to cut out a few of my questions and it also does not include the text in the form. Because I need these for medical documents I need all the notes and text included in the form.

    I hope there is a way of downloading a full pdf version

    I have downloaded your form as PDF and compared with the web form, here's what I can see:

    I noticed the text shows and all fields show properly, could you please try downloading your PDF fillable form again?
