Why is one of my elements not showing up on my form?

I have a drop down box of selections for my form (it's called mini session choices) and even though I can see it when I build the form, it's not showing up as an option on the form when I send the link to my clients.  

I have been having trouble with these forms lately showing up correctly, I just had another issue not too long ago and had to totally redo my whole form to get it to work.  Before I paid for a subscription, I had no issues whatsoever.  Now, I paid for a subscription and am having issue after issue.  Please help, it's slowing down my bookings.  Thank you.

Your Mini Session Choices: ( choose your mini session) field is conditional. However condition is set for the field that was removed from the form and because of that condition is not valid and your filed is hidden.

You have one more condition that is set to the field that was removed from the form.

To resolve you issue you need to disable or delete mentioned conditions.

Considering that you have embedded your form with full source code, you will also need to re-embed your form with new code, once you edit your form.
