    My form will not display on a mobile device variant.

    I was able to open the Page URL you provided using iPhone 6 - Safari Browser. It is working on my end. See image below:

    May we know what mobile that your form is not working? Also, please try re-embedding the form using the iFrame Method. Here's a guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code
