Need help getting data from one form to copy to another form

I have 2 forms

I set up the following link for my thank you form on the first form

Some data copies but no all

I notice that most of the fields from both forms were inversed and so the field data is not passed to the second form. I have fixed the fields in the URL. Please check with the following URL in the thank you page and let us know if the issue still persists.
