How to insert form information into subject line?

Bonjour comment insérer des informations du formulaire dans l'objet de l'e-mail qui est envoyé ? Merci !

Hi,

If you want to edit your email alerts, you can find it under Settings->Emails page. 

 

Below is a screencast on how I edited my email's subject line, you'll need to click the pencil icon at the left side to add form field information:
