Is this a phishing form?

I received this message from an old colleague through LinkedIn, but it seems like a phishing attempt.

We appreciate you reporting this to us. It's good that you have consulted us first before filling the form and submitting it. I have suspended the account that owns the form because the owner is clearly offering money in exchange for personal information and we do not tolerate such. There's also a high possibility that once someone's information is provided, the owner of the form will continue to contact them through phone or email to collect more information. Aside from that, after further investigating, the owner doesn't seem like he/she is from Amazon Inc. based on the information he/she has in his account.
