Help changing form design. Builder view does not match preview.

I am trying to change the design of my form.  Everything looks great in the builder view, however when I click the preview of the form, it does not look the same.  It has remnants of the old form.  I would like the appearance in the preview to match the appearance in the builder view.

If I understand you correctly, the form looks great in the form builder but it is not in the preview feature of the form. I cloned your form and tried using the preview, I was not able to replicate the issue. I also made a small changes just to see if the preview button is updating the form in the form builder. I was able to do that without any issue. See screen animation below:
