I can't login as my account got autosuspended

    My username is VernonPackard and I've been autosuspended.  I've tried to reset my password and have the instructions sent to my e-mail address but that isn't coming through.

    Upon examination of your account, I found that it got autosuspended because your form "Simple Job Application Form" had a field named "codename" and that triggered our automatic form review systems for suspicion of violating our terms of use.
