I checked your form and it looks like you were able to set up two different types of bullet points:

For some reason, Jotform could not display a disc for the bullet ● just like it is in the Word document; in Jotform the item list shows a circle instead. Remember the circle is for the secondary bullet list, not for the first item bullet list.

Part 1:
Here is the code that shows the bullet as circle for the first listing in my form above.
<ul>
<li>The IDOE is providing 9 Food Safety in Schools Trainings across Indiana.</li>
<li>The intention of this training is to provide school nutrition employees with up-to-date information on food safety.</li>
<li>Topics covered: food safety, prevention of foodborne illness, basics of microorganisms, how to keep a clean and sanitary school nutrition facility, process for foodborne illness prevention, and food safety programs in schools.</li>
<li>Attendees will receive credit toward their annual Professional Standards requirements.</li>
<li>All trainings are from 8:00am – 5:00pm local time, with a 1hr lunch break. Lunch will be on your own.</li>
</ul>
As my client's request, I have to manually the codes and use the disc ● images so that all text could be aligned just like it is in the item list.
Part 2:
Here is the code for the second listing in my form above:

I hope you now understand my question, and could be able to make the item list <ul> code in Jotform to display the main item list bullet as the disc ● just like they are displayed in the Word document (as the second listing in my form).

Please note that the issue should be fixed. I checked it on a newly created form and it works as expected. Please try and let us know if the issue persists?

Thank you for checking, but the problem has not been resolved.
I have just created another form but the bullets are still displayed as circles, please view the new form at https://form.jotformpro.com/71273658215963

Hello,
I see that the issue can't be seen on the builder, however, the list bullets are still displayed as empty circles on published versions.
I am sorry that my colleagues have missed that.
I am escalating the issue again to our developers.

We are again so sorry for this misunderstanding.
It was also fixed for preview now. Here is the view of the cloned form when preview the form.
