Maybe you enable the encryption option of that form, to disable encryption go to Settings -> show more options then select "no" to Encryption Form Data.

This seems to be a duplicate thread to https://www.jotform.com/answers/1136966/ which is just addressed. The issue is with the Preview before widget and updating the widget should resolve the issue.
