How can I show a header in the submissions?

I have included headers above sections of info being collected.  I would like the headers to be included in the submissions so I can see what the information relates to in the submitted form.

In the form submissions page, you just need to check the Show Headers and Text option:

In the notification email template, you need to add the headers manually. You have to add a row and add the header text there.
