My account suspended automatically by phising detection system [ASK]

I've read your email about the reasons why my account and form suspended. Here's the form

I'll answer the purpose of my form.

1. The purpose of the form: for getting competence data such: religion/bussiness/etc (there are 20 competences or categories) of 200 twitter user in Indonesia. We used it as our research about Indonesian credibiliy user based on its competence. So that, in this form, we used public perception or public labelling for giving competence of each user. The form will be used on our startup about credibility assesment on Indonesia. 2. I dont ASK username or password. I just ask name and email, its purpose for giving back the form result to the respondent if they want.
