Good afternoon.
Can you attach a PDF to a form? Meaning, can you have a checklist with terms and conditions and when all the fields have been checked, then someone can download a PDF.

Hey Heather, the first thing you need to do is to upload your PDF (or any downloadable file) elsewhere - e.g. a cloud file storage, your website's FTP, etc. Then get the direct download link and place it on your form (or even on your Autoresponder Email or Thank You page).
For this, you can use the Text Field which would allow you to add a clickable link on your form where users can download the document.
As an example, here's a .docx file that I uploaded on my test page. Now, I can use this same link and place it on a Text Field so users can download the file straight from my form.

To incorporate this with Conditions, all you need to do is craft your logic to something like this:
IF all Checkboxes are ticked THEN SHOW the Text Field
