Hello -
I went to our homepage, http://give2kids.com, and found our form looking like the attached screenshot.
It was completely altered from how we'd designed it.
I went to our JotForm account, pulled-up that form, and it was fine; as we'd designed it. So I went into the designer, made NO changes, and clicked on "Save". I then returned to our page, refreshed the screen, and the form looked as it should.
Why was the form messed-up? -

The published form on your website is not the updated version of your form.
I see you were using the full source code embed code of the form. When using the full source code embed, every time you make modification on the form, you will have to re upload or re publish also the form code on the website.
To fix, may I suggest you try first republishing the form on your website just using its default embed code instead.
