My form is unavailable

I am sending the link that I copy for the forms we have. When clients click the link, it says "form unavailable"

I have checked your account and found that it got autosuspended because your form "MPro Credit Restoration Application" is collecting passwords and sensitive information, in violation of our terms of use.
