"normally" is not working these days... probably new facebook updates that they failed to communicate to the rest of the world... but i'm desperate to get an image with my post!

have tried debugging via the facebook scraper tool and no joy...
not sure what's not working... have added image to social share widget (which i've never had to used in the past) and a form cover, which is usually what works...

Unfortunately, due to facebook restrictions on not allowing custom images on post anymore, you can't achieve this when sharing the direct link to your form.
A workaround is to use your form source code http://www.jotform.com/help/104-How-to-Download-a-Source-Code-of-your-Form.
Embed your source code on a clean HTML page. On top of the source code, add the Facebook meta tags from this page https://developers.facebook.com/docs/sharing/webmasters. Make sure to set your content values particularly the image as it is used as the thumbnail image on Facebook posts.
