I can send a test notification when from a form, but when I go to test from the actual form, it does not appear to submit the information to the email address I have supplied.
When I set up a new notification, shouldn't it submit to the email noted in the notification set up?

I checked the mail logs and I can verify that the email recipients of your form notification are receiving the email alerts.
However, I noticed that the Autoresponder email is not configured properly. Please take note that the Autoresponder email is the alert that the user will receive after completing your form.
The email recipient of your Autoresponder is set to "Member Type" dropdown. It is not an email field. This field will be used as the recipient so it should be an email field. Please do change it.
