Previously working form is no longer working

    I created a form named "IBI Associates Web Site Contact Form". It has previously been working, but now if I try to use it, I get the message "FORM NOT FOUND - this form is disabled". I went into settings, and verified that th eform is set as ENABLED. Not sure if I'm missing something, but hope you can help.

    I get that message when going to the Jotform page:

    When viewing the web page it's embedded in, it's just blank:

    Your account is currently suspended. The "Intake form" is asking for credit card information. Collecting credit card details using regular text field is not allowed here in JotForm. Please check our terms of use.

    You need to use one of our payment processors if you need to collect payments from your users.

    We can re-activate your account, but we need to delete the guilty fields. Also, we need your confirmation that you will not collect credit card details using regular text boxes.
