How can we get a voting form established on our website that allows people to vote and then pay for something we need- a dynamic form?

    We are a non-profit trying to launch an end of the school year campaign called 'You Pick the Flick' wherein parents can go to the website and vote for one of five films to be shown at our drive in this summer. We'd like the voting to be attached to an appeal that asks them to pay $10 for voting to put towards our general fund.
