How can we get a voting form established on our website that allows people to vote and then pay for something we need- a dynamic form?

We are a non-profit trying to launch an end of the school year campaign called 'You Pick the Flick' wherein parents can go to the website and vote for one of five films to be shown at our drive in this summer. We'd like the voting to be attached to an appeal that asks them to pay $10 for voting to put towards our general fund.

Please let me know if you can help- you were referred to me by our sales rep at Propel Marketing to help with this.

It is easy to build a form using JotForm. You can just drag and drop the required fields to the form. Please take a look at the following guide that can help you in creating a form.

    https://www.jotform.com/help/2-How-to-create-your-first-web-form

Also, you may receive the payments by integrating one of the payment methods on the form. Please refer to the guide below:

    https://www.jotform.com/help/165-Setting-Up-Your-First-Order-Form

Please give it a try and let us know if you need any further assistance. Since you mention that you are non-profit, you may apply for a non-profit discount from the link below:

    https://www.jotform.com/nonprofit/pricing/

Once the application is approved, you'll be receiving the links with discounted pricing to your email.

    Thanks!
