Moving a shared form to my personal folder

Hi,

    I recently paid someone on Fiverr to create 2 forms for me and I'm trying to now move then from the shared folder to my account/personal folder so that I have full control over the forms vs them.  I read that I cannot do this, it has to be completed by support.  Please advise moving these forms to my account in the folder - Master NMSP Forms Folder and the forms I need moved are "Bandwidth/Data Questionniare" and "Cloud/Hosted Phone System Questionnaire"

I have checked your account Nick77prob and see that there is only one form in it. However, as I check your recent edited JotForm, I see that it is associated on a different account diamondsquare4. If you have access to this account, we request you to login to this account and place your movement request. Otherwise, you may ask the person who created the forms for you.
