How can I create an e-mail alert button

Thanks for the answer David.  Is there a way to make it go to more than one e-mail address?  Maybe the respondent can pick from a menu which e-mail address to send the notification to?

I want to add a button to a survey that allows the respondent to click the button and it would automatically send an  alert to the surveyor that they want someone to contact them.  Is there a way to do this? 

You can set up notification emails that will be sent to a specific address each time the form is submitted:
