Why my form is not working?

Hi! We keep getting response that our orderform is not working and it keeps giving the 'NOT FOUND' result. I hope you can help asap.

Yes I can also access it. However, we are getting complaints from customers that they are receiving a 'NOT found' result when clicking the link.

Could you please give us more information, is that happening when they click Submit button or is your form embedded on a website?
