I am looking for a form that allows a person to fill out name, email and gets a free pdf document

Would that be the mini-subscribe form?  I want them to be able to automatically get the document once they fill in name and email and hit the button that would read "download" (In the template it reads "subscribe") sending the document to their email.  Is that possible?

You can indeed send users a PDF when they submit the form.  Set up an email field and then attach your PDF to an email autoresponder:
